-Make sure to first create the letter of entry in ERAS; you will need the letter ID# in order to be able to submit the form

-You will need to have your CV and personal statement ready to be uploaded along with the form. You will not be able to submit the form without those two documents.

-Please make sure to pick the correct specialty you are requesting the letter for

-Contact the clinical faculty assigned to the chair letter via email after submitting this form; they will require a meeting with you

-Filling out the incorrect information will delay your request for a chair letter, and you will be asked to resubmit the form

-Deadline for the student to request a chair letter will be September 1, 2024, 5:00PM MST
